I, hereafter referred to as “Employee”, agree to enter into a binding covenant with Safety-Flo, Inc., hereafter referred to as “The Company”.

Salary. Employee salary will be pro-rated based on Employee performance, durability, and longevity solely at the discretion of The Company.

Benefits. After a probationary period of six months, Employee may apply for health benefits.

By checking the box below, Employee affirms that all contract terms mentioned and unmentioned herein are acceptable, fair, and deserved. Employee agrees not to consult or communicate in any way (spoken, written, electronic, or otherwise) with any lawyers, their relatives, associates, friends or heirs, for the duration of this contract, which is indefinite and permanent.

I agree
